PLEASE NOTE: REPORTS/COMPLAINTS MAY BE SUBMITTED ANONYMOUSLY. IN THIS CASE, THE RESPONSE WILL BE MADE PUBLIC IN ACCORDANCE WITH THE PROCEDURE SET OUT IN THE RELEVANT GUIDELINES.
- The person wishing to make a complaint or report must indicate the date and type.
- The reporter must indicate which aspect the report/complaint refers to by ticking the corresponding letter.
- In field 5, the reporter must briefly describe the complaint/report.
- The reporter must provide their first and last name and sign the form; if they wish, they may leave this field blank and remain anonymous.
- Once the form has been completed, the interested party must place it in the appropriate box, or send it by email, fax, post, etc., addressed to the Steering Committee (options A to I) or RSG (options L to N).
- The Steering Committee/RSG will collect the complaint forms and, promptly and in any case within a maximum of 30 days, will decide on any necessary actions, notifying the interested parties who submitted the reports. In the event of corrective action, the Steering Committee/RSG will manage the situation in accordance with the procedure.
